Good lessons in considering system design with an engaging story for illustration.
George Schmitt |
a lot of techincal information that was somewhat difficult to read through to focus on the prociples of accident investigation
Russell Ring |
Lots of poorly asked questions. Hard to tell what the author was getting at with some questions. Be prepared to read this 3 times. Especially did not like questions 3, 8, and 9. some others too. Basic idea was good and the author made a good case for the failure analysis.

Entertaining and informative

I have a close friend that served on same class of sub and has a PhD in nuclear safety Hence some interesting discussions
William Ponton |
Excellent review of the accident and investigation.
George Currier |
Very interesting material!

Excellent course. Though I work within the US Navy submarine community and receive recurring training on the Thresher accident, I learned new and interesting information through this course.

I am an ocean engineer. The loss of the Thresher was a seminal event in the development of the ocean engineering discipline. This course provides an excellent overview of the accident, the likely causes and the changes to the US Navy submarine program as a result.
James Flannery |
The course was very well written and I learned a lot about safety systems and process safety considerations. Thank you!
Minna Leydorf |
Fascinating course that demonstrates how engineers can learn from failures and apply lessons learned from other engineering fields.
